Narrogin Cottage Homes is one of the largest providers of residential and community housing services in the Southern Wheatbelt.
We are currently seeking an experienced Registered Nurse to join our team as a Care Manager.
About the Role
The Care Manager is responsible for the management and supervision of appropriate quality care to residents by supervising the RN's to lead the care staff team; i.e. the team of direct care providers.
The role includes responsibility for:
Ensuring the correct clinical process is followed for all admissions, care planning, and re-assessments
Clinical governance adhered to
Progress notes and documentation are being completed accurately and timely
Scheduled care conferencing
Assist with clinical competencies in conjunction with the Clinical & Quality Manager and Staff Education Nurse
Correct and safe medication management in accordance with policies and procedures, and addressing any incidents that are found
Timely completion of incident reporting forms and any follow up action
Direct resident care/assessment, evaluation and directives for care staff to RN's
Portfolio management
Performance appraisals and management of staff
Contribute to continuous improvement
Manage staff rosters and workload
Ensure current knowledge and compliance with Aged Care Quality Standards
Requirements:
Registered Nurse with current AHPRA registration
Minimum 5 years' experience in aged care, with at least 2 years in a management role
Demonstrated knowledge of Aged Care Quality Standards and relevant legislation
Strong leadership and interpersonal skills
Excellent organisational and time management abilities
Proficiency in using electronic health record systems
Positive attitude that promotes the Narrogin Cottage Homes inc Mission, Goals and Values concepts
Police Check/Clearance not older than 3 months (or willingness to obtain one)
Desirable:
Post-graduate qualifications in aged care or health management
Experience working in rural or remote healthcare settings
Understanding of issues specific to rural aged care
Membership in industry reference groups, committees and associations
